How to Obtain Services
Referrals to our agency can be from your physician, hospital social worker / case manager, nursing home case worker, county case manager (MR/DD), minister, family, friends or self-referral.
A Registered Nurse will visit you in your home, hospital room or other private setting to provide a free consultation to discuss your needs and current situation. The RN will assist you in determining whether we can provide the services appropriate for you and develop your service plan based on your needs and your schedule.
